Patient Navigation

The patient navigator is a nurse trained to guide you through different aspects of the cancer care system. From sharing information about doctors’ offices, clinics, hospitals and outpatient centers, to patient support organizations and more, the patient navigator’s role is designed to support timely delivery of quality cancer care and ensure that patients, survivors and families are satisfied.


The Patient Navigator:

  • Coordinates patient care and education
  • Maintains communication with patients,survivors, families and healthcare providers to monitor patient satisfaction
  • Facilitates financial support and help with paperwork
  • Helps arrange transportation when needed
  • Refers to other services such as dietary, counseling and rehabilitation services
  • Directs patients to local resources and support
  • Ensures follow-up care and facilitates transition to survivorship
  • Accesses financial assistance for patient needs
